Patient's Consent
- I/We hereby willingly give my/our consent to undergo Homeopathic treatment under the direction of MindHeal Homeopathy.
- I/We have been explained the period and mode of treatment, alternative modes of therapy, implications, advantages and disadvantages of each of them, consequent benefits and sequelae of the treatment. I/We hereby accord my/our consent to this treatment of my/our own free act and will.
- After inquiry, questions and explanations, satisfying myself / ourselves, I/we have decided to consent and undergo the treatment.
- I/We have been explained that MindHeal Homeopathy is a company with a team of professional Homeopathic doctors and the management of the case will be done by a team of doctors.
- I/We state that no guarantee or warranty of results or cure and period of treatment has been given to me /us.
- I/We have been explained about the fees payable and the mode of payment.
- I understand / We have understood that fees paid are not transferable or refundable and MindHeal Homeopathy has the authority under its sole discretion to decide the norms for refund in case of natural or accidental death or death due to natural calamities.
- I/We have been explained that the patient will can opt to take a break in treatment, (e.g. in case of pregnancy or terminal illnesses requiring surgeries).
- I/We consent to the doctors conducting medical examination, audio/written recording of conversations, still photography, video recording and verifying investigation / laboratory reports and samples of the body under treatment and of the family member if required, provided that due care is taken while sharing my/our case studies for advancement of medical science and the benefit of mankind.
- I/We hereby give our consent to MindHeal Homeopathy to make use of our case to educate, train, demonstrate for the benefit of medical students, science and mankind.
- I/We undertake to comply with all the instructions given to me/us, verbal or written, during treatment.
- I/We have understood that past illnesses, if any, can get aggravated in certain cases during the course of treatment and result in delay in recovery of the illness being treated
- I/We have been explained and made understood that the treatment of mentally and physically challenged children is aimed at trying to enable them to lead an independent life, as far as possible, without any certainty of full recovery or cure.
- I/We confirm that all the things written herein above have been read and understood by me/us and I/we agree and accept all the clauses written herein above.
